Description: Hospice At Home has been providing care for individuals and families facing a life-limiting illness in Berrien, Cass, Van Buren and Allegan counties for more than 27 years. Patients, their families and caregivers receive compassionate care from the physician, nurse, hospice aide, social worker, spiritual counselor, bereavement coordinator and volunteer. Our team members go above and beyond to help patient's live the highest quality of life by providing symptom management, keeping pain under control and offering the opportunity for patients and their families to explore their spiritual and emotional concerns. Extending this care, Lory's Place, a bereavement and education center that builds hope and strength for every grieving family, is available to anyone in Southwest Michigan or Northern Indiana. In addition to traditional hospice care, Hospice At Home also provides Palliative Care physician visits for those living with a chronic, life-limiting illness. Palliative care may be used in addition to curative care of an illness. Enrollment in hospice is not a requirement for Palliative Care. We also offer relaxation and comfort choices such as massage, musical, and aroma therapies as well as pet visits and life reviews.
